Your front desk sends reminders, and replies start coming back. Some patients confirm. One asks which location the visit is at. Another needs a different day.

Hello Patient builds AI agents for healthcare. Our agents send appointment reminders by phone and text. They answer questions using information your practice supplies. The agent finds the patient's appointment in your scheduling system and confirms or changes it, inside the rules your practice sets.

This page covers what happens after the reminder goes out, from the patient's reply to the final appointment change or staff handoff.

What happens when a patient replies to an appointment reminder?

The agent answers the patient's question first. If the patient asks which location the appointment is at, the agent can give the address. The agent answers logistics questions like this one, such as addresses and hours, from information your practice supplies. Questions your practice wants a person to answer go to your staff, with what the patient asked.

The patient may then ask to change the appointment. The agent finds the visit in your scheduling system and changes it, inside the rules your practice sets. The change goes straight into your schedule, so the time the patient hears is the time on your calendar.

For the broader work of helping patients get to their visits, see our guide to reducing patient no-shows.

Text, call, or both: which patients get which reminder?

Your practice decides whether patients get a text, a call, or both. You can choose different channels for different groups of patients or appointments.

A text gives the patient a place to reply in writing. A phone reminder lets the patient ask and answer questions aloud. Your group might use text for one set of appointments and phone calls for another.

The same question can come through either channel. If a patient asks which location the appointment is at, the agent answers from the same practice information on either channel.

How far ahead should a medical practice send appointment reminders, and how many?

There is no single reminder schedule that fits every medical group. Your practice sets the timing and number of attempts around its appointments and rules.

Hello Patient lets the practice configure the channel, timing and number of attempts. Your practice decides when the first reminder goes out and how much time passes before another attempt.

Give the patient enough time to act on the reminder. If your practice asks patients to give a certain amount of notice before changing an appointment, set the reminder timing with that notice in mind.

Which confirmation calls can your front desk keep?

Staff handle the reminder conversations your practice wants a person to take. For text questions your practice sends to staff, see the outbound patient intake guide.

Your practice can also keep certain confirmation calls with the front desk. Your practice might have its front desk call the patients who have asked to speak with someone at the office.

What has to be true for a text appointment reminder to be HIPAA compliant?

A text appointment reminder can be HIPAA compliant. HIPAA permits appointment reminders as part of treatment without a separate HIPAA authorization. See the HHS guidance on appointment reminders. That permission does not mean every texting setup is compliant.

Your practice still needs reasonable safeguards and a way to honor reasonable requests for confidential contact. For phone messages, HHS advises limiting the detail left on an answering machine. Review the reminder wording and the patient's contact preferences before outreach starts.

The HIPAA minimum necessary rule has exceptions, including disclosures to the patient and disclosures to another healthcare provider for treatment. Those exceptions do not remove the need for safeguards. HHS explains the exceptions here.

Your practice also sets consent and opt-out handling for the outreach. The outbound patient intake guide covers related questions. STOP ends Hello Patient outreach immediately.

Hello Patient is HIPAA-compliant, SOC 2 Type 2 certified, and signs a BAA with every account. Read more about our approach to HIPAA compliance. This is general information, not legal advice.

What does the front desk get back after reminders go out?

Your front desk gets the requests your practice has chosen to keep with staff. A patient might reply, “I need to speak to someone at the office before I confirm.”

A request that needs a person can reach staff as a message or ticket. On a voice call, a live transfer is also available. Your practice decides how text requests reach staff.

Our agents work with the EHR, practice management and CRM systems healthcare already runs on. Your practice decides which appointment actions the agent can take.

For an example of our agents handling booking outside office hours, read the Carbon Health case study.

What should you ask a patient appointment reminder software vendor?

Ask the vendor to show what happens when a patient replies with a question instead of yes or no.

Start with “Which location is this at?” Then ask to move the visit. Have the vendor show you the answer, the appointment the agent found, the change in the scheduling system, and what the patient hears or reads after the change succeeds.

Then ask for a change your practice keeps with staff, such as a late change, and watch what reaches your front desk.

Already have a reminder tool you want to keep? Bring that setup to the conversation about the AI patient scheduling agent.

Frequently asked questions

Can a medical practice send appointment reminders to a shared family phone?

Your practice needs to follow the patient's contact preferences and watch what information the reminder reveals on a phone other people use. When you set up Hello Patient reminders, check whether the patient has asked you to use a private contact method. HHS says providers must accommodate reasonable requests for confidential communications, such as using another number. If the patient asks you to stop using the family phone, update the reminder setup before the next outreach. Staff need to review those instructions because a shared number alone does not tell you what the patient has agreed to receive there.

Which medical appointment changes requested by reminder text need staff approval?

Your practice decides which appointment changes need staff approval. Hello Patient uses those choices to decide which changes the agent can make and which requests go to your team. You may keep late changes or requests involving a fee with staff, so someone at the office can discuss the possible fee with the patient. For those changes, the agent finds the right appointment and sends the request to your team with what the patient said.

What happens if a patient never answers an appointment reminder text: does the agent call?

The agent calls after an unanswered text when your practice has chosen phone fallback. Hello Patient follows the channels, timing, number of attempts and stopping point your practice sets for that outreach. Your setup could send a text and then place a call, or stick to texts. If the patient still does not respond, the reminder sequence ends at the stopping point your practice chose. Your team can decide that sequence before outreach starts and know when the agent will stop trying.

Does a patient have to log into a portal or download an app to confirm a medical appointment?

No. The agent can confirm a visit through a phone or text conversation without making the patient log into a portal or download an app. Hello Patient's agent identifies the patient and finds the correct visit before it acts. That matters when a patient has more than one upcoming appointment and needs to know which one they are confirming. Your practice decides what else the reminder asks the patient to do before the visit.

Can a multi-location medical group run one appointment reminder setup with different hours and rules per location?

Hello Patient can use different hours for each location, and your rules for changing appointments can differ by practice or by provider. For example, one office might open at 8 a.m. while another opens at 9 a.m.; a reply asking when the office opens needs the hours for the patient's location. The agent uses the information and settings that apply to the patient's own visit. When your hours or rules change, your team updates the information and settings the agent uses for reminders.

How does a medical practice stop patients getting two reminders when its scheduling system already sends one?

Your practice needs to coordinate the systems that send reminders before the new setup goes live. Hello Patient can suppress its own appointment-change confirmation when the scheduling system sends that confirmation. Your practice also sets which reminders Hello Patient sends and when. Staff need to review the other reminder systems and decide which messages stay on. After an appointment change, check which confirmation the patient receives. Then review the separate reminders that are still scheduled to go out before the visit.
