Why some primary care patients never reach your portal's intake form

Your front desk may be helping a patient find a portal invite in an old inbox. The next caller may be stuck at a password reset. A daughter may be trying to arrange her father's visit through an account she cannot access.

Hello Patient builds AI agents for healthcare. The agent can collect the registration details needed for scheduling by phone. It can find or create the patient record and book under your practice's rules without making the patient log in to a portal first.

The general comparison of intake agents and portal forms covers the two approaches. This page covers which patients get stuck before the form, what the agent can handle, and what to count before you try another registration route.

How many patients on next week's schedule can use the portal?

Your portal enrollment number does not tell you how many patients on next week's schedule can actually get to the form.

Your enrollment report counts people who signed up. Your schedule shows the patients who need to do something before they come in. Look at both.

Nationally, 65% of individuals accessed an online medical record or patient portal at least once in the past year in 2024, according to ASTP/ONC Data Brief 77. That figure measures annual access. It does not tell you what share of your primary care panel enrolled or completed a form.

Which patients need help before they can reach the form?

Start with the account problems your team hears about.

A returning patient may have an invite tied to an old email address, while a new patient may be opening the portal for a previous practice. Someone on a recall list may reach the password reset screen and stop there.

Caregiver access deserves its own count. A daughter who books her father's appointments may need proxy access to complete his portal form. The setup and permissions vary by portal and practice, so check what your offices require.

A phone conversation gives the patient another way to provide registration details. A caregiver still has to meet your practice's identity and authority requirements. The agent uses the identity checks your practice sets. An unavailable portal account does not waive those checks.

Hello Patient is also SOC 2 Type 2 certified.

What changes when your offices use different portals?

You have to treat each office's portal and booking setup separately.

If your group acquired a practice and kept its system, a patient may need help finding the portal for the office where she is booked. The front desk has to figure out which office's portal she needs before walking her through an invite. Count those calls separately from password resets. A new password for the wrong portal will not get the patient to the right form.

The agent follows the location, provider, and appointment-type rules your practice has set. A demonstration needs to use the actual office and visit type you want to support, including which plans that office accepts. Ask us for a free demo agent to try it with that office.

Hello Patient integrates with the EHR, practice management, and CRM systems healthcare already runs on.

Can patients arrange an annual wellness visit without logging in?

Yes, a patient can call and provide the scheduling details needed to book.

The agent finds an existing patient record or creates a new record for a new patient. It then books an available appointment under your practice's rules. Because the agent looks for the existing record before it creates one, a returning patient keeps one record.

Your practice can also use outbound voice or text within the campaign and consent scope it has configured. That gives an eligible outreach list another registration route.

The agent tells the patient whether your practice accepts their plan, collects the plan details and adds them to the appointment, so your billing team has them before the visit.

The portal keeps its other jobs, such as results, messages and consent forms; the general comparison covers what stays there.

What should you count before choosing another intake route?

Count where patients are getting stuck.

Pull next week's schedule and separate new and returning patients. For each group, count how many have a portal account, how many can access it now, and how many have finished the form. If you do not know whether someone can get in, mark that as unknown instead of counting the patient as reachable.

Then sort the access problems by reason. That may be an invite the patient cannot find, a password reset, caregiver permissions, or the wrong office's portal. The completed-form count tells you what happens after patients get through. The access count shows where registration may be stopping before they see the first question.

If an intake vendor gives you a completion rate, ask what the denominator is. Does it include everyone invited, only people who logged in, or only people who started? A strong result among people who started the form can still leave your account-support calls untouched.

What would make a phone-registration pilot worth continuing?

A useful pilot starts with the account problems you counted.

If most calls concern one office's portal, start there. Ask to see a returning patient lookup, a new patient booking, and a returning patient whose portal invite went to an old email address. Those examples tell you more about your registration workflow than a booking demonstration with a perfect record.

For your group, the question is whether more patients can finish registration before their visit without needing the desk to walk them through an account.

Frequently asked questions

What does an AI intake agent do when a primary care patient calls asking for portal help?

Hello Patient's agent keeps the visit moving by phone. It collects the scheduling details, finds or creates the patient's record and books under your practice's rules, with no portal login. It leaves a note on the appointment with the portal problem the caller describes, such as a missing invite or a password reset that failed, so your team has it before the visit. Questions about a test result go to your clinical team with the patient's details.

What happens if a patient has already filled out half the portal form?

The patient can still get the visit booked without finishing the form first. Hello Patient's agent finds or creates the patient's record, collects the scheduling details your system requires and books under your rules by phone. It leaves a note on the appointment that the patient started the portal form, so your team sees that with the booking. It also tells the patient whether your practice accepts their plan and adds the plan details to the same note.

How does an AI intake agent find a returning primary care patient's record after a migration?

Hello Patient's agent looks the patient up in the system your practice runs now, typically by phone number and date of birth, so an account in the old portal does not get in the way. It finds the patient's existing record and books the visit straight into your current schedule, so the time the patient hears is the time on your calendar. In a migration pilot, include returning patients whose portal accounts were in the old system.

What happens when a primary care patient's portal invite went to an old email address?

Hello Patient's agent books the visit by phone, so the patient does not have to wait for the invite. It collects the scheduling details and books under your practice's rules. It also leaves a note on the appointment with the new email address the patient gives, so your team has it before the next portal invitation goes out. Include an old email address in your pilot examples, so you can see the booking and the note your team receives.

How does an AI intake agent check a primary care caller's identity without a portal login?

Hello Patient's agent uses the identity checks your practice sets. It finds the patient's record, typically by phone number and date of birth. For a new patient, it creates the record with the name, date of birth and contact details your system requires. An unavailable portal account does not waive those checks. A caregiver also has to meet the identity and authority requirements your practice sets before the request can move forward.

How can a primary care practice measure portal-support calls after adding an AI intake agent?

The agent gives patients another registration path, and your practice can track what it changes. Use the same call categories before and during the pilot: password recovery, missing invites, caregiver access, and wrong-office portals. Track these alongside registration calls handled by Hello Patient's agent. Compare similar periods and account for the number of scheduled patients, so a quieter week does not look like improvement by itself. Also count how many patients finished registration by phone before their visit.
