After-hours medical answering service: what should happen to each call

After the office closes, patients still call to book visits, check office details, or reach someone on your team. Some calls can be finished right there. Others are requests your practice wants a staff member to handle.

Hello Patient builds AI agents for healthcare. The agents answer after-hours calls using the rules your practice sets. They can book an allowed visit, answer office questions, or send a request to the staff member or team you chose. If you're still comparing coverage options, start with the guide to AI answering services for medical practices. This page covers what should happen to each type of call and what your team should test before going live.

What should happen to each kind of after-hours call?

Give each kind of call one clear next step and one destination.

A request assigned to a staff member goes to that person or team. A routine new visit is booked into an open slot your practice allows. An office question can be answered from the information your practice gave the agent.

A patient may call to ask where to park for a morning visit. The agent can answer that question and finish the call.

For refill requests, choose the staff destination through your prescription refill routing instructions.

What happens when a caller triggers the emergency response?

When a caller describes one of the emergency red flags your practice set, the agent follows the response your practice prescribed and escalates the call immediately.

For the full emergency-call route and how to test it, read the urgent-call guide.

Which requests should go to the on-call provider?

Send the on-call provider the request types your practice assigns to them. Other requests should go to the staff who own that work.

Take an insurance question. The agent answers "Do you accept this plan?" from the information your practice has given it. It also collects the plan details and adds them to the appointment note, so your billing team has them before the visit.

The same rule applies to other requests. Write down who should receive each type so you can test the route before launch.

Can a patient book a new routine visit after closing?

Yes. The agent books it into an open slot your practice allows.

The agent finds the patient's existing record first, so a returning patient keeps one chart. For a new patient, the agent creates a record. Your practice decides which locations, providers, visit types, and booking windows the agent may use.

The booking goes straight into your schedule, so the time the patient hears is the time on your calendar.

Hello Patient integrates with the EHR, practice management, and CRM systems healthcare already runs on.

At Carbon Health, Hello Patient handled after-hours calls and sent self-booking links. Of the patients who received those links, 40% booked. Read the Carbon Health case study.

What should staff see in an overnight request?

Staff should be able to see what the patient asked for, what the agent did, and who owns the next step.

Decide where that record should go and what it should contain. It should include the caller and contact details, office, reason for calling, action taken, destination, and responsible team.

For example, "appointment help" does not tell the morning team much. A better record names the office and visit type, says whether the agent booked the visit or took the request for your team, and names the team that should pick it up.

Hello Patient is HIPAA-compliant, SOC 2 Type 2 certified, and provides a BAA on every account. Read more about its privacy and security commitments.

How should after-hours rules work across locations and time zones?

Set the hours, time zone, and after-hours instructions for each location. The call should follow the rules for the office the patient is trying to reach.

Review each office's normal hours, booking rules, and staff destinations. If locations share a team, put that in the instructions. If they use different teams, make that clear too.

One office may be closed while another office two time zones away is still open. A patient calling the closed office should hear that office's hours and follow that office's after-hours route.

What should we ask to see in an after-hours demonstration?

Ask to see a normal after-hours call from start to finish, including the record your team receives afterward.

Start with a routine new visit. Ask the agent to find an allowed slot, complete the booking, and show you the appointment in your schedule.

Then test a normal request your practice wants a staff member to handle. The agent should capture the request, send it to the assigned destination, and record what it did.

Make a request that your front desk is supposed to handle the next morning. Then read the record as that front-desk employee. You should be able to tell what the patient asked for, what happened overnight, and what your team needs to do next.

Find out whether Hello Patient fits your practice. Book a call, and ask us to demonstrate your proposed after-hours booking and staff-request routes. On that call, ask for a free demo agent to hear how it would sound for your practice.

Frequently asked questions

What happens to a patient's non-emergency request when the assigned staff destination is unavailable after closing?

The next step should be the backup route your practice chose. With Hello Patient, you decide that route and who owns it before the agent goes live. If nobody answers a voice transfer, for example, the agent saves the request for the team you named. The record should show that the request was saved, not that a person was reached. Test the backup route so your team can see exactly what the agent does next.

Can a patient call after hours to cancel or move tomorrow's appointment?

Yes, by phone or text. Hello Patient's agent checks the patient's identity, finds the appointment, and cancels it or moves it to a new time inside the rules your practice sets. The change goes straight into your schedule, so what the patient hears matches your calendar. Website chat answers questions and books visits. Changes to an existing appointment run by call and text after the identity check. Use a test appointment to watch the change complete.

What can an AI agent tell patients about when practice staff will call back?

Hello Patient can tell patients the callback window your staff have agreed to. That way, the callback time a caller hears is one your team will keep. If the responsible team has committed to returning these calls within a set window, you can give the agent that wording. If there is no agreed window, the agent tells the patient it has taken the callback request for your team. Before launch, compare the words the patient hears with the response time your team has agreed to.

Is voicemail enough for our practice after hours?

It may be, if all you need after hours is a message for the morning team. Hello Patient's agents can also book an allowed visit or send a request to the staff route your practice has chosen. If your practice needs an action before the office reopens, voicemail alone will not finish it. With the agent, a new patient can book an allowed visit that night instead of leaving a message. Decide what each request type needs, then test that path.

How should an AI receptionist handle a temporary closure at one medical office?

Hello Patient should follow the temporary instructions you set for that office. Give the agent the exact dates and hours, the office answer patients should hear, and the staff destination to use during the closure. If one location closes for an afternoon, for example, test one call during the closure and another after normal instructions return. Decide who puts the change in place and who checks that the normal rules are restored.

How can practice staff tell whether an on-call handoff connected a call or captured a request?

The record should say whether the handoff connected a voice call, sent a message or ticket, or saved a callback request. Ask Hello Patient to show the result of the exact on-call route you plan to use. Your practice sets that route, the destination that receives the call, and when your nighttime rules allow a voice transfer. Review a test record, and agree with your team on what each result means before staff rely on the records.
